NJSL Position on Submitting Social Security Numbers
In response to privacy concerns, effective immediately, any newly registering player may choose to submit ONLY the last 4 digits of their social security number on both the player registration form AND on the supporting documention.

In other words, you can complete the player registration form with only the LAST 4 DIGITS of your social security number, and you can WHITE out the first 5 digits (thereby showing only the last 4 digits)  on any photocopied document as proof of your social security number. 


YOU MUST STILL PROVIDE PROOF OF YOUR SOCIAL SECURITY NUMBER IN THE FORM OF A COPY OF YOUR SOCIAL SECURITY CARD,  PAYSTUB, TAX FORM, W2, ETC., THOUGH YOU CAN WHITE OUT THE FIRST 5 DIGITS.

If you do not provide proof of social security number, you are Not Registered and are not eligible for insurance.

Remember you MUST BE registered properly, or you jeopardize your team's results and are not eligible for insurance.
